Who will do the assessment?

A key worker will be allocated to do the assessment.
This can be:

  • A social worker or member of the social work team.
  • A district or practise nurse from your GP surgery.
  • An Occupational Therapist.
  • Or another care professional who has been trained to assess the person’s needs.
