After the assessment
The person should receive a copy of the outcome of the assessment and how needs may be met. There should be a review date, this is usually around six months after the original assessment was completed. If the person disagrees with the outcome of the assessment they can ask for a further discussion of need.
If the person’s circumstances have changed they can also ask for further discussion or re assessment.
The person must be told about any waiting lists for the care they have been assessed for.
After an assessment the person might not receive all the care they need at the same time. Services are provided on the basis of need.
Any emergency care is dealt with separately for example at weekends, evenings and public holidays. Care support after leaving hospital should be arranged before the person is discharged if it is essential.
