Why use supported self-management?
Supported self-management is an integral part of several important Scottish government strategies and reports including:

These focus on prevention, anticipation and supported self-management with the patient/client firmly at the center of all decisions, taking charge of their health and maintaining their independence for as long as possible. Health and social care professionals work in ways to ensure people with long-term conditions have the knowledge, skills, confidence and support to self-manage.
